Before long, the expansion clip at the bottom of the bolt simply slid over the end of the expansion cone, and the bolt failed. The hole that the bolt was placed in literally got bigger and bigger. It was completely ineffective at resiting the expansion of the bolt. Note in the picture above that there is no longer an expansion clip on the bolt--the clip is still inside the hole.
